UPSEE UG Application Form
1. Distribution and Submission of UPTU SEE OMR Application Forms 2013
1. OMR application form with Brochure can be obtained from 1st March 2013 to 21st March 2013 ( both days inclusive ) from Head Post Offices and Mukhya Dak Ghar ( MDGs ) of Uttar Pradesh.
2. The Cost of Information Brochure and application form including examination fee will be as under :-

3. Completed application form ( OMR Type ) duly filled must reach to the following address upto 25th March 2013 ( 5.00 P.M. ) by Speed Post / Registered Post only. The application form delivered by any other means shall not be accepted. The application forms received in the University after the last date will not be accepted.

2. Submission of UPSEE 2013 Online Application Form
1. UPTU SEE Application Procedure 2013 : The online application can be made from 10.A.M., 28th February 2013 till 10:00 P.M. of 21st March 2013 through the website http://upsee.nic.in or http://uptu.ac.in. For assistance ( in case of any difficulty ) in filling up ON-LINE application form, candidate may contact the felicitation centres at various cities. The list of felicitation centres shall be made available on website http://upsee.nic.in.
2. The applicants should submit all the details as per their eligibility while filling the online application form. After filling online application form; you are required to review all the entries filled by you by clicking on “review button” before submitting it. On submission of application, a registration number will be generated. You are required to note this number for all future correspondence.
3. Now you may pay the required examination fee through either bank challan or credit / debit card / internet banking.
- If you are opting to pay through bank challan, click the required link ( download e-challan ) to generate the challan in triplicate. The challan will contain your credentials along with the required fees. You should take the challan along with your signature and the requisite fees to the designated bank for the payment on next day ( Day 2 ) of filling the application form, if the form is filled till 5 P.M. In case application is filled after 5 pm, the candidate is supposed to take the duly signed challan for payment on Day 3 to the Bank. The bank will provide you a transaction number, make sure that the transaction number is filled up correctly at the space provided on the challan. To generate the confirmation page ( if the payment is made through e-challan ) visit the website http://upsee.nic.in or http://uptu.ac.in again on further next day ( Day 3 or Day 4 ) and click the link “Generate the Confirmation Page ( e-challan )”. Fill up registration number and transaction number ( provided by bank ) alongwith other required details to generate the confirmation page.
- If you are making payment through credit card / debit card / internet banking then click an appropriate link and make the payment. All VISA / Master cards are accepted. You should be ready with your credit, debit card / internet banking details. Make the payment and generate the confirmation page.
4. Take the printout of Confirmation Page, paste your recent passport size photograph, put your signature & thumb impression ( left thumb ), parent / guardian signature at the space provided on the confirmation page. Post the duly signed completed confirmation page along with copy of challan ( if the payment is made through bank challan ) so as to reach the following address on or before 25th March 2013. Please do not forget to mention your mailing address, contact phone number at the space provided on the confirmation page.

3. UPSEE 2013 Examination Fee
The examination fee for online application is as follows :

Please note that the bank may additionally charge service charges plus applicable taxes while making the payment through Credit / Debit card or Internet Banking.
4. UPTU SEE 2013 Admit Card
- Admit card will be sent by post ( Speed Post / Registered Post ) and available online to every candidate. However, Downloaded admit card shall be treated as valid admit card. The candidates are advised to check the details of admit cards with regards to their category, sub category, gender, rural weightage, Name, Father’s Name etc. as mentioned on the Admit Card. In case of any discrepancy, the candidate may report to the examination centre superintendent for such correction on Form no. 09 on the day of examination.
- A nodal centre in each city of examination shall be established to issue Duplicate Admit Cards ( in case of non-receipt of admit card ) on 20th April, 2013 from 10.00 A.M. to 5.00 P.M., but not on the dates of examination to these candidates. No candidate will be allowed to appear in the examination without admit card.
- To get duplicate admit card, the candidate should contact the nodal centre situated in the city of examination, which he / she has opted as first choice. Such candidate should carry the following to the nodal centre :
- Two passport size photographs ( Black & White ) similar to that pasted on the application form.
25.00 as Fee for duplicate admit card.- The OMR application form number / Online Application’s Reference Number.
- Proof (receipt of speed Post / registered Post) of having submitted the application form.
- If any candidate is unable to receive or download his / her admit card, he / she should contact the helpline or send an email as indicated on the website latest by 17th April, 2013.
- A plea that a candidate failed to receive the information for appearing at the Examination will not be accepted as a ground for the refund of fee or any other redressal. UPSEE 2013 may withdraw the permission to appear in UPSEE 2013 granted by mistake to a candidate who is not eligible to appear at the UPSEE 2013 even if an Admit Card has been issued and produced by the candidate before the concerned Centre Superintendent at the Examination Centre.
- In case of any reported correction, the final corrected admit card will have to be downloaded from website www.upsee.nic.in / www.uptu.ac.in before the examination date.
- Since successful candidates are required to produce the Admit Card at the time of counselling and admission, it should be carefully preserved until the admission process is completed.

3. UPTU SEE 2013 Admit Card
