2. Computer downloaded forms should be accompanied by an additional amount of Rs. 600/- (Prospectus price) in addition to the Entrance Test fee. Total Rs.1350/ for SC/ST candidates/handicapped candidates and Rs. 1600/- for others.
3. Computer Form, not tagged, be presented/sent in loose form alongwith Application Form-A.
4. Attendance-cum-Identification Form and Admit Card duly filled be presented/ sent in loose form alongwith Application Form-A.

B. The following documents be attached with Application Form-A.

5. Attested copy of the Reserved Category Certificate, if applicable.
6. Attested copy of Residence Certificates Part B alongwith affidavit Part A.
7. Three passport size photographs pasted with gum respectively in the designated space on the Application Form-A, Admit Card and Attendance cum Identification Form. One unattested photograph be pasted on the Computer Form in the designated space.
8. Mode of submission of Application Form either personally or through Registered Post.
9. Self-addressed envelope of the size 9"44" with postal stamps affixed worth Rs. 27/-.

Important Notes:

1) For Late fee Candidates: Admit Card and Roll No. alongwith the centre will be issued on the spot to candidates who submit Application Form-A with relevant late fee (complete in all respects) at:

General Branch, Admistrative Block,
Guru Nanak Dev University,
Amritsar.

Such candidates shall apply personally and not through post.

2) Mode of payment of fee including late fee shall be by bank draft drawn in favour of Registrar, Guru Nanak Dev University, Amritsar payable at Amritsar or by cash on the counter of the Cashier, Guru Nanak Dev University, Amritsar.

3) Bank Draft to be tagged in front of Application Form-A.

4) Attested copy of the reserved category certificate from the competent authority to get the benefit of reservation as well as of the fees, if applicable.

5) If a candidate does not submit his/her application on the prescribed Application Form-A in ORIGINAL/DOWNLOADED (not a copy or photocopy or typed copy), his/her Application Form for appearing in the Entrance Test shall be summarily rejected. He/she shall not be allowed to take the test under any circumstances whatsoever.

Exact venue of examination will, however, be intimated to the individual candidates by the Guru Nanak Dev University, Amritsar. The University reserves the right of not holding examination at a centre where the strength of examinees opting for the centre is less than 80 and 20 in case of A and B above, respectively.
