Application form along with the Information Brochure can be obtained by post by sending a written request through Registered Post alongwith MICR demand draft of Rs. 800/- (Rs. 550/- for SC/ST Candidates) drawn in favour of the Director, IMS, BHU, payable at Varanasi along with two self addressed white paper slips of 6 cm x 10 cm in size from the Office of

The envelop should be clearly superscribed “Application for B.A.M.S. / B. Pharm (Ay) Entrance Test – 2008, BHU”.

The request for application form by post must reach on or before 01.03.2008. The application form can also be obtained on cash payment of Rs. 750/- (Rs. 500/- for SC/ST candidates) from the counter of the main Post Office on the BHU Campus between 20.01.08 and 17.03.08. The last date for submission of completed application is 19.03.08 upto 5.00 PM. Application Form for MD/MS Courses: Application form can either be downloaded from BHU website ( www.bhu.ac.in) or can be obtained from the office of Director, IMS, BHU, Varanasi by sending a written request through Registered Post along with a demand draft (MICR) of Rs. 100/- drawn in favour of the Director, IMS, BHU, payable at Varanasi along with two self addressed white paper slips of 6 cm x 10 cm in size. The envelop should be clearly superscripted “Application for MD (Ay)/MS (Ay) Entrance Test – 2008, BHU”. The request for application form by post must reach on or before 17.03.2008. The application form can also be obtained on cash payment of Rs. 100/- from the counter of the main Post Office on the BHU Campus between 20.01.08 & 31.03.08. The last date for submission of completed application accompanied by DD/Banker’s Cheque of Rs. 1000/- (Rs 750 /- for SC and ST) drawn in favour of DIRECTOR, IMS, BHU is 04.04.08 upto 5.00 PM.
