By Cash: On payment of Rs. 1000/- (Rs. 600/- for SC/ST candidates) from the following branches of BANK OF BARODA.
1. Varanasi – (1) BHU, (2) Ram Katora, and (3) Orderly Bazar Branches.
2. Delhi–(1) Parliament Street, (2) Shakti Nagar, and (3) Nehru Place Branches.
3. Lucknow – Charbagh Branch.
4. Kolkata – College Street Branch.
5. Hyderabad – Hyderabad Main Branch, ABIDS Circle.
6. Pune – Station Road Branch
7. Bhopal – TT Nagar Branch
8. Jaipur – DCM Branch.

Application Form can be obtained from Bank of Baroda (BHU Branch, Varanasi only), up to Thursday, and 06.03.2008 till 4:00 PM. By Postal Channel: From the Office of the Director (PMT-Cell), Institute of Medical Sciences, Banaras Hindu University, Varanasi-221 005 by sending a written request through Registered Post along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 1060/- (Rs. 660/- for SC/ST candidates) of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi. Application form will be sent by Speed / Registered Post. However, Institute will not be responsible for any loss or postal delay. The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate’s name and address on the back of DD). Two self-addressed, preferably typed, white paper slips of 10 cm ´ 6 cm size MUST also accompany it. The ENVELOP containing requisition for PMT/PAT application form should be super scribed “Request for Application for PMT/PAT-2008, BHU”. Last date of requisition for PMT/PAT form by post is Wednesday, the 20th February 2008. Incomplete/non-cashable Draft (under amount/unsigned/ without Bank Code No. & Seal) shall not be accepted. Note: (A) Separate Bank Draft will be required for each application form. (B) M.O./I.P.O./Cheques shall NOT be accepted under any circumstances. Application Forms from the Office of the Director (PMT-Cell), Institute of Medical Sciences, can be obtained by POST only and not in person. In case forms are not available at Bank of Baroda Branches, the same can be obtained by post from Director’s Office up to 20th February, 2008. Important Dates:

Application forms received after the last date shall not be accepted. Foreign Nationals are not required to appear in the PMT examination for MBBS course. They may seek admission through the Ministry of External Affairs, Govt. of India, New Delhi, as per the rules laid down for the purpose and their nomination must reach by Wednesday, the 16th July 2008.
