JSSU UGET 2013 MBBS Application Form

A candidate desirous of appearing in the JSSU UGET 2013 entrance test for Undergraduate Medical Course is required to complete the prescribed application form and submit to the University in the following ways :

1. A candidate is required to download the prescribed application form from the University website : www.jssuni.edu.in and submit the completed form to the University on or before the scheduled date.

The downloaded and printed application form should be sent along with a processing fee of 2,000/- only by way of DD favouring “JSS UNIVERSITY FEE ACCOUNT”, MYSORE, payable at Mysore along with the documents listed below. *

2. Candidates may also obtain the application form along with the Brochure by sending a application fee of 200/- only by way of DD favouring “JSS UNIVERSITY FEE ACCOUNT”, MYSORE , payable at Mysore. The duly filled application form should be sent to the University along with documents listed below enclosing DD for 2,000/- favouring “JSS UNIVERSITY FEE ACCOUNT”, MYSORE, payable at Mysore.

Enclosures with Application

* Attested copies of the following documents have to be enclosed along with the application form and kindly arrange the documents in the following order.

1. S.S.L.C. / Equivalent examination marks card.

2. P.U.C. / Equivalent examination marks card / Proof of having appeared the PUC II / 10+2 / HSC or equivalent examination.

3.Marks cards of B.Sc., / Equivalent examination of all the three years ( in case of graduates )

4. Conduct / Character and Transfer certificate issued by the college last studied.

5. Date of birth certificate if not mentioned in the SSLC / Equivalent examination marks card.

6. Caste / community certificate

7. Transfer certificate for Karnataka board candidates.

8. Migration Certificate issued by the Board / University in case of candidates other than Karnataka State education board.

9. Six recent, identical passport – size colour photographs with name and date. ( In addition one copy of the photo should be pasted on the application form in the space provided ).

10. Foreign Nationals seeking admission should obtain eligibility certificate issued by JSS University, Mysore.

11. Processing fee of 2,000/- only by way of DD favouring “JSS UNIVERSITY FEE ACCOUNT”, MYSORE, payable at Mysore. The candidate should write name , address and “Application for admission to MBBS courses” on the back side of the Demand Draft.

Note :

Candidates who are likely to appear or who have appeared for qualifying examination i.e. PUC – II / 10+2 / HSC or equivalent but whose results have not been declared will also be considered to appear for JSSU UGET 2013, However it is responsibility of the applicant to ensure the HSC / 10+2 / PUC marks card copy is sent to the University specifying the TAT No. ( Test Admission Ticket Number ) before Last Week of May, 2013 failing which, the tie in the rank will be declared on the basis of the University rules, which shall be binding on the candidate.

Issue of JSSU UGET MBBS TAT 2013 ( Test Admission Ticket )

1. Candidates appearing for JSSU UGET 2013 will be issued a Test Admission Ticket ( TAT ) for identification. Candidates are required to bring the Test Admission Ticket in original ( Candidate Copy ).

2. The Test Admission Ticket gives the details of test centre assigned to the candidate. The candidates are assigned a test centre decided by the JSS University authority during the application processing, itself and no requests for change in Test centre would be considered.

3. TAT Cards will be dispatched to the candidates from the University on schedule. The TAT Card will indicate the Roll Number & Examination Centre allotted to the candidate with its address.

4. The candidate must not mutilate this TAT Card or change any entry, made therein after the University authorities have authenticated it.

5. JSS University is not responsible for any loss in transit or for incorrect address given by the candidate in the Application Form.

6. Candidates are advised to preserve their TAT in good condition till admission in the College is over.

7. Please recheck the website for any changes / updates before leaving your hometown for the test.

8. In case a candidate does not get his / her TAT Card ( Hall Ticket ) by 23rd May, 2013 by post / courier, he / she should contact University office immediately. Arrangement will be made for the issue of duplicate TAT Cards at the ‘Help Desk’ established on the previous day of the examination at respective examination centers. In such a situation, candidate should bring two photographs which are identical to the photo affixed to application form, along with his / her proof of submission / posting of application form or school / college identity card. In case no proof is submitted duplicate TAT card will not be issued.

A candidate desirous of admission to the B.Pharm, Pharm D. and Pharm D. ( Post Baccalaureate ) course is required to complete the prescribed application form and submit in either of the following ways :

• A candidate is required to complete the prescribed application form and submit the same to the University on or before the scheduled date. The downloaded and printed application form should be sent along with a processing fee of 1,000/- only by way of DD favouring “JSS University Fee Account, Mysore,” payable at Mysore along with the relevant documents.
• Candidates may also obtain the application form along with the Brochure by sending a processing fee of 200/- only by way of DD favouring “JSS University Fee Account, Mysore,”, payable at Mysore.

The duly filled application form should be sent to the University along with relevant documents. With a processing fee of 1,000/- only by way of DD favouring “JSS University Fee Account, Mysore,” payable at Mysore.

A candidate desirous of appearing in the JSS University Entrance Test – 2013 for Undergraduate Dental Course ( BDS ) is required to complete the prescribed application form and submit to the University in the following ways :

• A candidate is required to download the prescribed application form from the University Website : www.jssuni.edu.in and submit the completed form to the University on or before the scheduled date . The downloaded and printed application form should be sent along with a processing fee of 1,000/- only by way of DD favouring “JSS UNIVERSITY FEE ACCOUNT”, MYSORE, payable at Mysore along with the documents listed below.*

JSSU BDS Entrance Selection Procedure 2013

1. Submission of the application form for entrance test / admission by post or in person along with Photographs and other enclosures on or 17th June, 2013.

2. Selection will be made on the merit based on marks secured in Qualifying examinations ( PUC II / 10+2 / HSC or equivalent ) in relevant subjects conducted by various Board / Universities and JSSU entrance examination.

3. The application downloaded from the Website of the University should be accompanied by a DD for 1,000/- drawn in favour of “JSS UNIVERSITY FEE ACCOUNT, Mysore” payable at Mysore.

4. The entrance test will be followed by counseling and admissions on Last Week of June, 2013. The counseling process will be announced on the Website : www.jssuni.edu.in.

5. The Exam Center will be at MYSORE center only.

6. Admission of selected students to courses and colleges allotted during counseling session.
